EDITS+ Documentation Differences

EDITS+ includes a placeholder code, DocDiff, that coders can assign to mark visits with incomplete information. Reports summarizing this information are available in the operational report options. The documentation differences feature allows reporting about what could have been charged, if visit information had been complete, helping to track revenue lost due to uncodable or undercoded visits.

Documentation Difference Reports

Reports about documentation differences, used to summarize documentation issues and to track revenue loss, are available from the Operational option in the Reports menu.

 
Documentation Feedback
 

Provides a list of charts where the problem warranted a higher code than the visit documentation supported. For example: Insufficient review of systems, or EKG interpretation not documented.

 
 
Documentation Feedback with Variance
 

Same information as Documentation Feedback report, but includes potential revenue loss.

 
 
Facility Documentation Feedback
 

Same as the Documentation Feedback report, but for the Facility. For example: Name of medication not provided.
